If you’re nominating a family, please fill out the form to begin the process of applying for yourself or referring a family you know who is navigating cancer while raising keiki ages 18 and under.

When filling out the form, please indicate whether you’re applying or nominating someone for a Gift Card / Resource Giveaway or an ‘Ohana Fun Day (or both).

This is the first step in the process. Once we receive this initial application, we’ll send you a more detailed form to help us learn more about the recipient, their family, and their journey. This information allows our board to thoughtfully review each nomination and determine how we can best support the family, whether through gifting gifts/gift card/ and resources or by curating a meaningful and joy-filled ‘Ohana Fun Day experience for their family.

At this time, we’re only able to provide gift cards / give aways / ‘Ohana Fun Day for families currently living in Hawai‘i.
